Distraction osteogenesis of the mandible has become an established technique that is useful in the management of growing patients with craniofacial microsomia. 1 When children attain skeletal maturity, it may be necessary to correct problems in occlusion and facial dysmorphism with a combination of Le Fort I osteotomy, bilateral sagittal split osteotomies of the mandible, and genioplasty. Sagittal split osteotomy (SSO), introduced by Rittersma and Van Gool in 1979, is an excellent technique for accessing various pathologies located deep in body or ramus of the mandible and in close proximity with the inferior alveolar nerve . This technique can also avoid various complications associated with the conventional approaches.

It was then decided to perform Obwegeser’s bilateral sagittal split osteotomy with a setback of 8 mm of the mandible. This would result in perfect occlusion of the teeth along with establishment of a pleasing facial profile. 15 May 2015 Surgical access for the sagittal split osteotomy is performed in the standard fashion. During sagittal split ramus osteotomies of the mandible, the inferior alveolar nerve can be directly damaged by a burr on a rotary drill, a blade on a reciprocating saw, or a chisel used to complete the split. The nerve can also be compressed during osteotomy stabilization (i.e., plate and screw fixation). Most maxillofacial deformities can be managed with the three basic osteotomies discussed in this article: the LeFort I type osteotomy, the bilateral sagittal split osteotomy of the mandibular ramus, and the horizontal osteotomy of the symphysis of the chin (osseous genioplasty).

A sagittal split osteotomy is an oral surgery procedure that is done to correct any serious misalignment of the upper and lower teeth. The bone on the sagittal — or side — of the lower mandible of the jaw is cut on each side to form a split. This allows the front part of the mandible to slide backward or forward until the teeth are aligned.
